Request a Traffic Crash Report

Effective January 1, 2007, the Missouri State Highway Patrol began charging a $3.25 fee for each basic motor vehicle crash report copy. Requests for report copies of crashes that occurred in 1997 or later can either be forwarded to the Highway Patrol headquarters of the troop where the motor vehicle crash occurred, or to the Traffic Division. (A listing of troop headquarters, their mailing address and telephone number can be obtained by visiting the Home page of this Website.) However, requests for report copies of crashes that occurred in 1996 or earlier, certified crash reports, crash report photographs or crash reconstruction reports must be forwarded to the Traffic Division.

To obtain a copy of a basic motor vehicle crash report investigated by the Highway Patrol, wait 10 days from the date of the crash investigation then complete the request form attached here. Forward the completed form to the appropriate troop headquarters or Traffic Division along with your check or money order made payable to DPS - Missouri State Highway Patrol. Cash payments are not accepted and will be returned. Requests submitted to the Traffic Division may be mailed to Missouri State Highway Patrol, Traffic Division, P.O. Box 568, Jefferson City, Missouri 65102, Attention: Traffic Crash Reports or faxed at (573) 751-9921, Attention: Traffic Crash Reports.
