About This Kit
Your kit for recording identification records for your child. Complete the information below and keep it in a safe place. To report a missing person in Missouri, call local law enforcement or dial 911.
Identification Records
Record the following information for your child:
- Full name, address, and phone number
- Nickname
- Date of birth, sex, and race
- Place of birth (city, state, hospital)
- School(s) attended (names, addresses)
- Parent's or guardian's name, address, and phone number
Physical and Personal Characteristics
Appearance
- Complexion: Light, Medium, Dark, Ruddy, Freckles, Dimples, Other
- Hair: Black, Blonde, Brown, Red, Other
- Eyes: Black, Blue, Brown, Gray, Green, Hazel
- Build: Slender, Medium, Heavy
- Devices: Glasses, Contacts, Prosthetic, Other
Height and Weight
Record at age, feet, inches, and weight.
Distinguishing Features
- Birthmarks and their location
- Scars and marks (pierced ears, tattoos, bites nails, scars, marks and location)
- Disabilities (limp, speech impediment, etc.)
Medical Information
- Allergies
- Eating Habits: Favorite foods, foods disliked
- Teeth: Permanent teeth, braces, filings (location), caps (location), missing teeth (location)
- Broken Bones: Location
- Blood Type
- X-Rays: On file at (location)
- Serious illnesses requiring special medication
Additional Records
- Names and addresses of doctor and dentist where medical and dental records are on file
- Hobbies, favorite pastimes, and places person likes to visit
- Friends and acquaintances who might provide a "lead" on the missing person
- Any other identifying information
- A recent photo
Fingerprint Records
Fingers must be fully inked and rolled. Improperly taken prints are of no value for identification.
Record fingerprints for:
- Right hand: Thumb, Index, Middle, Ring, Little
- Left hand: Thumb, Index, Middle, Ring, Little
- Right four fingers taken simultaneously
- Left four fingers taken simultaneously
- Both thumbs
